CoC Program Application Information
The U.S. Department of Housing and Urban Development (HUD) releases a Notice of Funding Opportunity (NOFO) for the Continuum of Care (CoC) Program.
The FY2024 CoC Program Competition has ended. The CoC’s final submission to HUD can be found under the FY2024 & Fy2025 CoC Program Application heading.
Instructions for the FY2025 CoC Program Application will be posted here as soon as they become available.

Scorecard Committee Materials
The Scorecard Committee, composed of representatives from Regional Committees and staffed by NCCEH, meets each year to create the scorecards used for scoring new and renewal CoC Program project applications.

CoC Builds
On May 16, 2025, HUD reissued a special Notice of Funding Opportunity (NOFO) called CoC Builds via grants.gov. This NOFO targets efforts within the CoC to address and reduce the number of people experiencing homelessness by adding new units of permanent supportive housing (PSH) through new construction, acquisition, or rehabilitation through the CoC Builds awards under the CoC Program.
Please review the CoC Builds NOFO page if you are interested in applying. Be sure to read the NOFO in its entirety to understand the full scope of the application requirements.
Based on the FY2025 Final Pro Rata Need of the NC Balance of State CoC, the maximum amount a single project may apply for under the CoC Builds NOFO is$9,000,000.
Match requirement: Match of no less than 25% cash or in-kind, or a combination of both, is required according to 24 CFR 578.73

Intent to Apply
Eligible agencies interested in applying are required to submit an Intent to Apply form by Wednesday, May 28th at 12 PM (Noon). To access the intent to apply form, go to this link.
Eligible agencies must have an active and up-to-date account with sam.gov at the time of application.
Applications for this CoC Builds NOFO are in e-snaps. Please note, this is a different system than the previous CoC Builds NOFO which was through grants.gov.
Applications
Applicants must submit pdfs of their project applicant profile, final application from e-snaps and all required attachments to the CoC at NCCEH by Thursday, June 5th at 12 PM (Noon) via this link. Applications received by the deadline will be scored. The CoC will not score applications received after the deadline.

Applicants without e-snaps access need to follow the instructions provided by HUD for e-snaps setup.
Once the Project Applicant Profile has been created, all applicants will need to create the Project Application by following detailed instructions to be posted on the CoC Builds NOFO page.
Application Requirements
Project Applicant Profile (pdf) from e-snaps: Applicants must complete an Applicant Profile in e-snaps, export it as a PDF, including all attachments, and submit it via the Smartsheet link provided above for the June 5th deadline. Non-profits must include their IRS 501c3 final determination letter. All applicants must attach their organization’s Code of Conduct.
Project Application (pdf) in e-snaps: Project applicants must complete a Project Application in e-snaps, export it as a PDF, and submit it via the Smartsheet link provided for the June 5th deadline. DO NOT SUBMIT IN E-SNAPS for the June 5th deadline. Project Application Detailed Instructions are to be posted on the CoC Builds NOFO page.
Required attachments:

- Form HUD-2991 must be completed and dated between May 12, 2025, and June 26, 2025 – Applicants must submit a certification by the jurisdiction in which the proposed project will be located so that the applicant’s project application for
funding is consistent with the jurisdiction’s HUD-approved consolidated plan. This attachment should be uploaded into the Project Application in e-snaps in the attachment section. - Code of Conduct must be uploaded in the attachment section of the Project Application in e-snaps, in addition to the Project Applicant Profile in e-snaps.
- Non-Profits only: Non-profits must upload their IRS 501c3 final determination letter into the Project Application in e-snaps in the attachment section, in addition to the Project Applicant Profile in e-snaps.
- Letter of Commitment, Contract, Other Federal Written Commitment in response to Rating Factor V.A.1.h. – Coordination with Housing Providers, Healthcare Organizations, and Social Service Providers. This attachment should be uploaded into the Project Application in e-snaps in the attachment section.
- Evidence of Site Control: Evidence includes a deed or lease for new construction or rehabilitation, or a purchase agreement for acquisition. This attachment should be uploaded into the Project Application in e-snaps in the attachment section.
- Form HUD-2991 must be completed and dated between May 12, 2025, and June 26, 2025 – Applicants must submit a certification by the jurisdiction in which the proposed project will be located so that the applicant’s project application for
Application Optional Attachments:
Opportunity Zone: Submit Form HUD-2996. If the project will be in an opportunityzone, applicants can submit this optional attachment. This attachment should be uploaded into the Project Application in e-snaps in the attachment section.
Completed Financial Feasibility Analysis: If the project has a completed financial feasibility analysis conducted by a Housing Credit Agency, you are encouraged to attach the analysis in the attachment section of the Project Application in e-snaps.

ESG PROGRAM INFORMATION
ESG Program Overview
The Homeless Emergency Assistance and Rapid Transition to Housing Act of 2009 (HEARTH Act) amended the McKinney-Vento Homeless Assistance Act, including major revisions to the Emergency Shelter Grants program, now the Emergency Solutions Grants (ESG) program.
The NC Department of Health and Human Services (NC DHHS) administers non-entitlement ESG Program funds in North Carolina. More information about the ESG Program is available on the NC DHHS website.

ESG Program Annual Competition
The Emergency Solutions Grant (ESG) Program competitive application cycle occurs on an annual basis. ESG Program funding is administered by the State and is overseen by the NC ESG Program Office in the NC DHHS Division of Aging and Adult Services.
The ESG Program application includes two parts: a Regional Application completed by NC Balance of State Continuum of Care (NC BoS CoC) staff and a Project Application completed by each agency seeking funding.
There are two steps to the application process: First, agencies within the NC BoS CoC must submit Project Applications to the CoC for review by the Project Review Committee and approval by the Steering Committee. Project Applications approved by the Steering Committee are then, as a second step, submitted by applicants to the NC ESG Program Office at NC DHHS for their review and final approval.

ESG Program Application – RFA Issued June 11, 2024
On June 11, 2024, the North Carolina Department of Health and Human Services (NC DHHS) released a grant opportunity for non-profit agencies and units of local government to provide services to assist individuals and families experiencing or at risk of homelessness. This is the CY2025 ESG Program annual allocation for agencies providing eligible activities for ESG Program funding, including Street Outreach, Emergency Shelter, Rapid Rehousing, Homelessness Prevention, and Homeless Management Information System (HMIS) services.
Agencies applying for NC ESG Program funding from NC DHHS for services to be provided in any of the 79 counties within the North Carolina Balance of State Continuum of Care (NC BoS CoC) must follow the instructions linked below.

In this year’s competition, NCCEH staff will complete the Regional Application and Regional Budget on the NC Balance of State CoC’s behalf. Regional Committees do not need to complete an individual Regional Application for their Regional Committee.
