Continuums of Care
In order to coordinate activities to meet the needs of individuals and families who are experiencing homelessness and to apply for federal funds, community partners come together to form a Continuum of Care (CoC). The CoC Program is designed to promote communitywide commitment to the goal of ending homelessness by providing funding for efforts by nonprofit providers and State and local governments and promoting access to and effect utilization of mainstream programs by homeless individuals and families.

North Carolina has twelve Continuums of Care:
- Asheville/Buncombe County
- Chapel Hill/Orange County
- Charlotte/Mecklenburg County
- Durham City/Durham County
- Fayetteville/Cumberland County
- Gastonia/Cleveland/Gaston/Lincoln Counties
- Greensboro/High Point/Guilford County
- Northwest NC
- Raleigh/Wake County
- Wilmington/Brunswick/New Hanover/Pender Counties
- Winston-Salem-Forsyth County
- North Carolina Balance of State
Homeless assistance providers apply through their Continuum of Care for HUD funding for transitional housing, permanent supportive housing, rapid re-housing, supportive services, and HMIS projects.
In North Carolina, the structure of CoCs varies amongst our communities. Some cities and counties assign staff to assist the CoC, while other communities ask organizations to volunteer their time.
