Tracking Discharges from Adult Care Homes into Homelessness

Description
The North Carolina Coalition to End Homelessness (NCCEH) has received reports of individuals being discharged from adult care homes into homelessness. Due to changes in the ability to bill Medicaid, there are some adult care homes that are at higher risk for discharging into homelessness.
In order capture data on these discharges, CHIN will be adding a field that will track if individuals have been in these adult care homes in 2012. It important for homeless providers to track this information so we can provide the State and local partners an on the ground picture and assist them in successful discharge planning.
Join this call to learn more about the issue that has the potential to create a new population of disabled homeless if transitions are not implemented thoughtfully.
