The National Human Services Data Consortium (NHSDC) is a collaborative organization that understands the importance of data quality and the value of using valid and reliable data for community planning and performance measurement. NHSDC has worked for over 10 years to deliver conferences that allow Homeless Management Information System (HMIS) professionals to discuss current challenges and best practices while developing a critical network of peer support.
 
The 2012 Conference will be held April 26 and 27, with a half-day tour of Chicago area Continuum of Care programs on April 25.

Presenters will discuss the VA’s recent NOFA to implement a Transition in Place model for homeless veterans through the Grant and Per Diem program. Among the topics discussed:
  • Can a Transition in Place model serve homeless people with high service needs?
  • How do providers decide who will be best served in this model?
  • What tools exist to help make these decisions?
  • What considerations regarding housing stock and single vs. scattered-site models are most important?
