NCCEH Dialogue Group

This is a regular meeting of the NCCEH SOAR Caseworkers Dialogue Group.
It is a caseworker roundtable to problem-solve and connect with others around the state who are working on SOAR cases.

The Scorecard Committee is responsible for creating the scorecard that is used to score project applications for Balance of State CoC funding.
 
This meeting will be held via conference call.  Dial-in instructions will be emailed to all registrants prior to the call.

Training/Webinar

This webinar will provide participants with an overview of the health home concept and explain the benefits of including supportive housing service providers in health home infrastructures. The Affordable Care Act includes a provision allowing states to experiment with Medicaid reimbursement for health home service delivery to chronically ill populations. Given that most supportive housing residents live with at least one and often multiple chronic health conditions, Medicaid agencies and health industry leaders should include supportive housing services as they implement the health home model.
