Regional Meeting Schedule

Piedmont Regional Committee meetings are held on the 2nd Wednesday of each month. Each quarter the meeting is held in person, hosted by a local provider agency. In-person meetings occur in March, June, September, and December each year. Please contact the Regional Lead to receive information about these meetings.

Piedmont Regional Case Conferencing is held on the 2nd and 4th Wednesdays of each month. Please contact the Coordinated Entry Lead to receive information about these meetings.

Coordinated Assessment

Each Regional Committee has a coordinated assessment plan to assess and refer people experiencing homelessness to local services and housing.

Coordinated Assessment Forms

PRC VI-SPDAT Single
To be used with any single adult household or any family unit who are all over the age of 18.

PRC VI-SPDAT Family
To be used with any multiple person household who has at least one member under the age of 18.

Greivance Form for Individuals

Any person who belives he or she has been adversely affected by an act or decision of Piedmont Region Committee Coordinated Assessment has the right to process a complaint or grievance.

Grievance Form for Agencies

Any agency who believes he or she has been adversely affected by an act or decision of Piedmont Region Committee Coordinated Assessment has the right to process a complaint or grievance.

HMIS Release of Information
All HOH's that complete a VI-SPDAT assessment and are entered into HMIS must review and sign this document in order to participate in Coordinated Assessment system.

To be used with each referral to a housing agency within the PRC.

Participant Contact Form
All participants in the BOS must fill out this form. Please send to the completed forms to the CA Lead.
