Balance of State Continuum of Care: 2016 CoC Application

The Continuum of Care (CoC) application is released by HUD on an annual basis. The 2016 CoC NOFA was released on June 28. The NOFA is available on the HUD Exchange website.

The Balance of State is required by HUD to post the full CoC application and project priority listing prior to submission in esnaps.

Intent to Apply Form

Balance of State agencies that are interested in applying for new (not renewal) funds in the 2016 CoC competition are asked to fill out the Intent to Apply form by Friday, July 15.

2016 Scorecards

The scorecards are used by the Project Review Committee to score and rank project applications. All applicant agencies should read the scorecards to ensure they understand the requriements, expectations, and priorities for project applications.

ALL applicants must complete this form. It is submitted to the State to determine whether projects are consistent with the goals of the State Consolidated Plan.

Forms for Regional Committees
New Project Review Form for Regional Committees
All project applications must be presented to their Regional Committees for review. For new projects, the Regional Lead must complete this form certifying that the application was reviewed and approved by the Regional Committee		 download form

 

Project Review Committee Materials

The Project Review Committee, composed of representatives from BoS Regional Committees and staffed by NCCEH, is responsible for scoring project applications and creating the ranked list of projects that is presented for Steering Committee approval.

Scorecard Committee Materials

System Performance Measures

As part of the 2016 application, CoCs were required to submit System Performance Measures to HUD through the Homeless Data Exchange (HDX). The measures allow HUD to assess CoCs' performance as a coordinated system on key indicators on ending homelessness.
